Fraternity & Sorority Life
- Grade Release and Non-Hazing Form
This form must be completed to add any new members or transfer members to your chapter roster and is due within 7 days of them joining your organization.
- Greek Community Standards Packet (Due Monday, November 23)
All chapters/colonies must submit a Greek Community Standards Packet. To learn more about the Greek Community Standards click here.
- Officer/Advisor Update
This form must be completed by all chapters/colonies at the beginning of each semester.
- New Member Education Plan
Required to be submitted by Panhellenic and IFC organizaitons two week sprior to the start of New Member Education.
- Good Standing Form
This form must be completed by all chapters/colonies at the beginning of each fall.
- Greek Report Form
This form must be completed by all chapters/colonies at the end of each semester.
- Roster Update
This form must be completed by all chapters/colonies at the end of each semester.
- Greek Emerging Leaders (GEL) Application
Every Chapter is expected to have at least one member participate, with a maximum of five members participating. Attendance is mandatory for all sessions, students will be allowed one EXCUSED absence, with documentation.
- GLI RSVP
Council Officers, Chapter Presidents and one additional chapter leader are required to attend GLI.
- Officer Training Day RSVP
Council and Chapter Officers are expected to attend Officer Trianing Day.
- Risk Assessment Guide
This is a resource for students who are planning events to review risk related to those events prior to hosting.
Events
- ON CAMPUS Event Authorization
This is done through the chapter's Cougar Connect page and must be submitted 14 days in advance for all on-campus events.
- OFF CAMPUS Event Notification Form
This form must be submitted 14 days in advance for all off-campus events.
- Student Liability and Release Waiver
If you are coordinating or providing transportation for an event, this form must be completed, or if you are hosting an event with some liability involved.
- Domestic Travel Waiver
Members must complete this waiver once per semester if they are traveling out of town with the chapter or to a chapter function.
